Cluster van differentiatie 138 (CD138), ook bekend als Syndecan-1, is een lid van de transmembraan heparansulfaat proteoglycaan familie die aanwezig is op het oppervlak van B-cellen tijdens de late differentiatie fase. CD138 fungeert als extracellulaire matrixreceptor en is betrokken bij vele cellulaire functies, waaronder cel-cel adhesie en cel-matrix adhesie. CD138 komt zowel in hematopoëtische als niet-hematopoëtische cellen tot expressie. In het hematopoëtische systeem labelt CD138 plasmacellen. Het is een uitstekende marker voor plasmacytaire differentiatie binnen het spectrum van hematologische maligniteiten. Onder niet-hematolymfoïde cellen wordt CD138-reactiviteit waargenomen in vele soorten epitheelcellen en stomacellen in zowel normale als tumorweefsels.

Anti-CD138 is used to differentiate marginal zone lymphoma from lymphoplasmacytic lymphoma. ALK+ Large B-Cell Lymphoma (LBCL) commonly stains positively for CD138, but not for CD20 and CD79a. Anti-CD138 reacts positively with HHV8-associated primary effusion lymphoma that lack B-cell markers. CD138 is also a useful marker for identifying and enumerating benign, reactive, or malignant plasma cells from the bone marrow biopsy samples.
